Hospice RN Case Manager

Job Summary
Direct patients to appropriate care levels by triaging symptoms, coordinating with providers, and arranging urgent care or emergency services. Maintain the high-risk patient database, review census weekly, and stratify cases in team meetings to define care needs and home visit frequencies. Place timely calls to assess health status, monitor treatment plan compliance, educate patients on disease processes, and ensure follow-up appointments to prevent readmissions. Document accurately in GMG systems, authorize HH/DME forms, and communicate pertinent findings to the clinical care team. Meet care coordination and nurse triage production goals while maintaining strict confidentiality.
Required Qualifications
- Must be state-licensed RN
- Minimum three years of RN experience
- Must have knowledge of medical terminology
- Must have knowledge of medical terminology, anatomy, physiology
- Must have a good understanding of documentation standards
- Must be able to work efficiently under pressure
- Requires manual dexterity

- Requires lifting up to 40 lbs without assistance

Desired Qualifications
- Graduation from an approved BSN program
- Experience in a clinic, doctor's office or hospital as a RN for at least three years
- Prior managed care experience
- Excellent customer service skills
- Must have excellent written and verbal communication skills
- Strong attention to detail and exceptional organizational skills
- Must maintain strict confidentiality of company, department, and patient information
- Skilled in use of computer/EMR systems
- Knowledge of Word processing software
- Knowledge of spreadsheet software
- Knowledge of Internet
- Knowledge of database software
