Job Summary
Provide skilled nursing care, including medication administration, for hospice patients in home, assisted living, and skilled nursing facility settings. Perform comprehensive assessments, coordinate care plans with families and caregivers, and monitor patient status to report changes to supervisors and physicians. Participate in weekly interdisciplinary team meetings and maintain all clinical documentation within required timeframes using the computer-based system. Submit time records and provide input for hospice aide evaluations as needed. Weekend and holiday coverage is available as required. Previous hospice experience is desired, with at least one year of RN experience needed.
Required Qualifications
- Graduate of an accredited RN program
- Registered Nurse Licensed in the state of employment
- At least 1-2 years of RN experience
