Job Summary
Provide skilled nursing care, administer medications per physician plans, and perform comprehensive assessments for hospice patients in homes, assisted living, and skilled nursing facilities. Coordinate care plans with families and caregivers, while observing and reporting physical, emotional, and psychosocial status changes to supervisors and physicians. Participate in weekly interdisciplinary team meetings and maintain all clinical documentation and time records in compliance with agency standards using the computer-based system. Submit required information within specified timeframes and provide input for hospice aide evaluations. Weekend and holiday coverage as needed. Requires RN license, 1-2 years experience, and previous hospice care preferred.
Required Qualifications
- Graduate of an accredited RN program
- Registered Nurse Licensed in the state of employment
- At least 1-2 years of RN experience
