Hospice RN Case Manager

Job Summary
Coordinate and manage care for terminally ill patients as Hospice RN Case Manager. Conduct initial and ongoing patient assessments, develop personalized care plans with patients, families, and the interdisciplinary team, and coordinate services to ensure continuity of care. Educate patients and families about disease progression, symptom management, and end-of-life options; provide emotional support and guidance. Monitor and manage pain and symptoms, maintain accurate HIPAA-compliant documentation, and participate in interdisciplinary team meetings to discuss care plans and progress. Demonstrate strong communication, collaboration, and compassionate, high-quality patient care in a healthcare setting.
Required Qualifications
- Education: Completion of an accredited Registered Nurse (RN) program
- Licensure: Current RN license in the state of practice
- Certification in Hospice and Palliative Care Nursing (CHPN) preferred
- CPR certification required
- Experience in hospice or palliative care preferred
- Strong clinical and patient care skills
- Proficiency in electronic medical records (EMR) systems
- Ability to work independently and as part of an interdisciplinary team
- Compassionate, empathetic, and dedicated to providing high-quality patient care
