Job Summary
Hospice Registered Nurse / RN Case Manager responsible for enabling patients to spend quality time with loved ones, building trusting relationships with patients and families, empowering caregivers through education on home-based care, and collaborating with the care team to provide comfort, peace, and dignity. Requires an active RN license, at least two years of nursing experience (hospice experience preferred), a valid driver's license, CPR/BCLS certification.
