Job Summary
Plan and direct hospice care by completing comprehensive assessments of patients and families, initiating care plans, and revising them as status changes. Supervise hospice aides, administer prescribed medications, and counsel families on nursing needs and activities of daily living. Coordinate patient care for assigned caseloads and facilitate efforts toward self-sufficiency or comfort care. Requires an active Massachusetts RN license, acute and home care experience, and CPR certification. Schedule is Monday through Friday, 8a-4:30p, with on-the-road duties and mileage reimbursement.
Required Qualifications
- One (1) to two (2) years of recent acute care experience in an institutional setting
- One (1) to two (2) years of recent experience in home care
- CPR certification
- Active and unencumbered RN license in Massachusetts
Desired Qualifications
- Bachelor's degree
- One (1) year of hospice care experience
