Hospice RN Case Manager- Hillsboro

Assume primary responsibility for patient and family caseload, including assessment, eligibility determination, admission processing, care planning, implementation, and evaluation. Obtain data from physical, psychological, social, and spiritual factors to initiate interdisciplinary interventions and coordinate with physicians and agencies. Provide regular visits to re-assess needs, update care plans, perform clinical pronouncements of death, and support families. Supervise and document Home Health Aide delivery of care per guidelines while maintaining accurate records and progress notes. Work Monday through Friday from 8 a.m. to 5 p.m. with no on-call rotation.
