Hospice RN Case Manager

Conduct comprehensive initial and ongoing assessments of patients and families to determine hospice needs, including complete physical assessments and histories. Develop care plans establishing goals based on nursing diagnoses, incorporating palliative actions and involving patients and families in planning. Initiate preventive and rehabilitative procedures, administer prescribed medications, and provide health care instructions and self-care teaching. Coordinate patient care for assigned caseloads, communicate with physicians regarding condition changes, and maintain accurate clinical notes documenting pain management and outcomes. Facilitate activities of daily living and emotional support through cooperation with the interdisciplinary group.
