Job Summary
Hospice RN Case Manager responsible for managing an assigned patient caseload within the Gloucester/Camden County, NJ region. Performs nursing assessments, planning, implementation and evaluation of care to maximize patient comfort, coordinates with attending physicians and the interdisciplinary team, communicates with physicians and agencies, documents in electronic medical records, participates in interdisciplinary meetings, and provides caregiver support and education. Role includes on-call coverage and involvement in bereavement and hospice orientation/training, with a starting salary of $85,250 and full-time benefits. Must be a graduate of an accredited nursing program, NJ-licensed RN, maintain good assessment/problem-solving skills, be able to travel to patient homes, and be fluent in English and Spanish.
Required Qualifications
- Graduate from an accredited School of Nursing
- Licensed in NJ as a Registered Nurse by New Jersey State Board of Nursing
- One year of RN experience (Hospice experience preferred)
- Must be a licensed driver with an insured automobile
- Must be fluent in English and Spanish
