Hospice RN Case Manager

Job Summary
Direct patients to appropriate care levels by assessing symptoms, appointing providers, and routing to urgent care or emergency rooms per triage protocols. Maintain accurate databases of high-risk patients, review census weekly, and stratify needs in team meetings to define home visit frequencies. Place timely calls to assess health status, monitor treatment plan compliance, educate on disease processes, and arrange follow-up appointments with primary care and other providers. Document findings in GMG systems, authorize HH/DME forms, and communicate pertinent issues to the clinical care team. Ensure patients have prompt hospital follow-ups to prevent readmissions while meeting care coordination production goals.
Required Qualifications
- Must be state-licensed RN
- Minimum three years of RN experience
- Graduation from an approved BSN program
- Must have knowledge of medical terminology
- Must have knowledge of medical terminology, anatomy, physiology
- Must have a good understanding of documentation standards
- Must be able to work efficiently under pressure
- Requires manual dexterity

- Requires lifting up to 40 lbs
- Close vision and ability to adjust focus
- Must keep all licenses and certifications current and in good standing
Desired Qualifications
- clinic experience preferred
- Prior managed care experience preferred
- Experience in a clinic, doctor's office or hospital as a RN for at least three years preferred
- Must have excellent customer service skills
- Skilled in use of computer/EMR systems
- Knowledge of Word processing software
- Knowledge of spreadsheet software
- Knowledge of Internet
- Knowledge of database software
