Hospice RN Case Manager - Facilities

Manage patient discomfort and provide symptom relief using specialized nursing skills under physician direction. Conduct initial and ongoing assessments of terminal diagnoses to evaluate physical, functional, psychosocial, and environmental needs. Collaborate with the Interdisciplinary Group to develop, implement, and update individualized plans of care, including preventive procedures and referrals. Educate patients, residents, families, and caregivers regarding disease processes, palliative interventions, and safety practices. Work Monday through Friday from 8 to 5 visiting hospice patients in facilities within South Pinellas County.
