Job Summary
Hospice RN Case Manager provides skilled nursing visits in patients' homes across Santa Fe and surrounding communities, managing pain and symptom control, administering physician-approved treatments, and coordinating care with the interdisciplinary team. Responsibilities include on-call rotation coverage, patient assessments, documentation in the electronic medical record, patient and family education, and advocacy for patient-centered end-of-life care. The role requires independent decision-making, strong communication, and the ability to work autonomously in the field while maintaining timely care and collaboration with IDT members.
Required Qualifications
- Active RN license in New Mexico
- At least 1 year of nursing experience (hospice/home health strongly preferred)
- Ability to work autonomously in the field with good time management
- Valid driver's license and reliable vehicle
- CPR certification preferred
