Job Summary
Conduct comprehensive assessments of hospice patients and families to determine care needs, then write and initiate patient-centered plans of care that include palliative nursing actions for pain and symptom management. Provide skilled nursing interventions, administer prescribed medications, and counsel families on meeting nursing and related needs while teaching self-care techniques. Coordinate patient care for assigned caseloads by communicating with physicians and the interdisciplinary group, ensuring equipment availability, and supervising LPNs and aides. Submit accurate clinical notes documenting condition changes and outcomes. Participate in on-call duties, attend interdisciplinary meetings, and actively contribute to quality assessment and performance improvement teams.
Required Qualifications
- Registered nurse with current licensure to practice professional nursing in the state
- Graduate of National League for Nursing accredited school of nursing
- Current CPR certification
- Licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
- Minimum of two years' experience, at least one of which is in the area of public health, home care, or hospice nursing
- Self directed and able to work with minimal supervision
- Demonstrates excellent observation, problem solving, verbal and written communications; nursing skills per competency checklist
- Shows ability to organize and prioritize workload independently
- Prolonged or considerable walking or standing
- Able to lift, position, and/or transfer patients
- Able to lift supplies and equipment
- Considerable reaching, stooping, bending, kneeling, and/or crouching
- Visual acuity and hearing to perform required nursing skills
