Job Summary
Hospice Registered Nurse / RN Case Manager delivering home-based hospice care in Saginaw, MI. Build trusting relationships with patients and families, empower caregivers through education on care, and collaborate with the care team to provide comfort, peace, and dignity. Requires an active RN license in the state of practice, at least two years of nursing experience (hospice experience preferred), valid driver’s license, and current CPR/BCLS certification. Full-time role with comprehensive onboarding and benefits; travel to patient locations as part of care coordination.
