Job Summary
Hospice Registered Nurse / RN Case Manager role delivering home-based hospice care. Build trusting, compassionate relationships with patients and their families; empower caregivers by educating them on care; collaborate with the care team to provide comfort, peace, and dignity. Requires an active RN license in the applicable state and at least two years of RN experience; hospice experience preferred. Must have a valid driver’s license and current CPR/BCLS certification.
