Job Summary
Hospice RN Case Manager coordinates patient care in Fresno/Clovis and surrounding areas, performing initial and ongoing comprehensive assessments of patients and families, developing and revising the care plan, administering medications and treatments as prescribed, counseling patients and families, teaching self-care, and ensuring continuity of care. Responsibilities include documenting clinical notes, communicating with physicians and the interdisciplinary team, participating in on-call duties, supervising hospice aides when required, and maintaining a safe environment. Requires California RN licensure and CPR certification; 1 year of hospice/public health/home care experience preferred. The role offers a competitive hourly compensation ($50-$55/hour) and comprehensive benefits.
Required Qualifications
- Graduate of a National League of Nursing accredited school of nursing
- Current license in California and current CPR certification
- RN license with minimum of 1 year of experience in hospice/public health/home care or a BSN with required experience
- Must be a licensed driver with insured automobile
- Self-directed with minimal supervision
- Excellent observation, verbal and written communication, problem solving, nursing skills
- Ability to walk, stand, lift and transfer patients and equipment; good vision and hearing
