Hospice RN Case manager

Guide a multidisciplinary team of LPNs, aides, social workers, and chaplains to create personalized care experiences for patients in Northeast Indiana. Champion each patient's unique needs and wishes while developing and implementing care plans that prioritize quality of life and comfort. Build meaningful connections by providing counseling and emotional support, and document each patient's journey with care. Travel throughout the region to deliver healing in homes, assisted living facilities, long-term care facilities, and hospitals. This full-time role requires an active RN license, valid driver's license, and one year of hospice experience, with BLS certification and palliative medicine background preferred.
