Hospice RN Case Manager

Lead a multidisciplinary team of LPNs, aides, social workers, and chaplains to create personalized care experiences for patients in their homes, assisted living facilities, long-term care facilities, and hospitals throughout Northern Indiana. Champion each patient's unique needs, preferences, and wishes while developing and implementing care plans that prioritize quality of life and comfort. Build relationships that matter by providing counseling and emotional support, and document each patient's journey to capture progress and precious moments. This full-time role requires an active RN license, valid driver's license, and one or more years of hospice experience, with flexible scheduling available Monday through Friday and weekends as needed.
