Job Summary
Conduct high-quality patient care using clinical nursing knowledge, physical assessments, and procedural skills at the patient's residence. Develop and oversee plans of care under physician direction while performing initial and comprehensive assessments on all patients. Deliver counseling and education to patients, caregivers, and families as indicated in the plan of care, ensuring appropriate interdisciplinary involvement for optimal outcomes. Maintain clear, concise, and accurate clinical documentation through continuous visits and updates to the care plan. Requires an active Iowa RN license, CPR certification, and hospice experience; eligible for CHPN certification within 24 months. Hourly compensation with comprehensive benefits including medical, dental, vision, 403(b), and PTO.
Required Qualifications
- Active Iowa RN license
- Active CPR certification
- Hospice or palliative care experience
- Current driver's license
- Automobile liability insurance
- Ability to travel between multiple locations within your region
- Home access to the internet
- Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment
Desired Qualifications
- Two (2) years of verifiable experience working as a nurse (RN/LPN), preferred
