Hospice RN - Case Manager

Job Summary
Provide high-quality hospice nursing care in patients' place of residence (home or facility) as an RN Case Manager. Responsibilities include using clinical nursing knowledge and assessments to deliver comprehensive patient care, developing and overseeing plan of care under physician direction, performing initial and ongoing patient assessments, ensuring interdisciplinary involvement, delivering clear clinical documentation, and counseling/educating patients and families as outlined in the plan of care. Travel within the region to support patients, continuous interdisciplinary assessment, and maintaining up-to-date care plans to meet patient-specific needs.
Required Qualifications
- Active Iowa RN license
- Active CPR certification
- Hospice or palliative care experience
- Two (2) years of verifiable experience working as a nurse (RN/LPN)
- Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region
- Home access to the internet
- Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment
