Job Summary
Conduct high-quality patient care in the patient's place of residence using clinical nursing knowledge, physical assessment, and procedural skills. Perform initial and comprehensive assessments on all patients and deliver continuous visits with interdisciplinary assessment. Develop and oversee a plan of care under physician direction, ensuring appropriate interdisciplinary involvement and identifying required services. Provide patient, caregiver, and family counseling and education as indicated in the plan of care while delivering clear, concise, and accurate clinical documentation. Maintain an active Iowa RN license, CPR certification, and current driver's license with the ability to travel between multiple locations. Obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment. Required Qualifications
- Active Iowa RN license
- Active CPR certification
- Hospice or palliative care experience
- Current driver's license
- Automobile liability insurance
- Ability to travel between multiple locations within your region
- Home access to the internet
- Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment
Desired Qualifications
- Two (2) years of verifiable experience working as a nurse (RN/LPN)
