Job Summary
Hospice RN Case Manager providing high-quality patient care in the patient’s place of residence (home, long-term care, assisted living, or hospital). Responsibilities include performing comprehensive assessments, developing and overseeing care plans under physician direction, ensuring interdisciplinary involvement, delivering documentation, counseling patients and families, and updating care plans to meet patient-specific needs. Requires active Nebraska RN license, CPR certification, hospice/palliative care experience, ability to travel within the region, and CHPN within 24 months. Required Qualifications
- Active Nebraska RN license
- Active CPR certification
- Hospice or palliative care experience
- Two (2) years of verifiable experience working as a nurse (RN/LPN), preferred
- Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region
- Home access to the internet
- Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment
