Hospice RN - Case Manager

Job Summary
Deliver high-quality patient care using clinical nursing knowledge, physical assessments, and procedural skills at the patient's residence. Perform initial and comprehensive assessments, conduct continuous visits, and oversee the development of interdisciplinary plans of care under physician direction. Provide counseling and education to patients, caregivers, and families while ensuring accurate clinical documentation. Maintain active Iowa RN licensure, CPR certification, and a valid driver's license with the ability to travel across Storm Lake, Pocahontas, Sac City, and surrounding areas. Obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months.
Required Qualifications
- Active Iowa RN license
- Active CPR certification
- Hospice or palliative care experience
- Current driver's license
- Automobile liability insurance
- Ability to travel between multiple locations within your region
- Home access to the internet
- Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment
Desired Qualifications
- Two (2) years of verifiable experience working as a nurse (RN/LPN), preferred
