Job Summary
Complete comprehensive nursing assessments to evaluate patients' physical, emotional, and psychosocial needs. Develop and manage individualized plans of care in collaboration with physicians, social workers, and the interdisciplinary team. Monitor patient progress, adjust care plans, and administer medications while managing pain and end-of-life symptoms. Serve as the primary clinical resource for patients and families, providing education on disease progression and symptom management. Build trusting relationships through active listening and help loved ones navigate goals of care and end-of-life planning. Coordinate transitions of care, participate in regular interdisciplinary team meetings, and fulfill on-call responsibilities. Maintain accurate EMR documentation and ensure compliance with Medicare Conditions of Participation and state regulations. Requires current unrestricted RN license, minimum one year of nursing experience, and ability to travel throughout the assigned service area.
Required Qualifications
- Current, unrestricted Registered Nurse (RN) license in the state of practice
- Minimum of one (1) year of nursing experience
- Valid driver's license
- Reliable transportation
- The ability to travel throughout the assigned service area
Desired Qualifications
- Previous hospice, palliative care, home health, oncology, or case management experience
- Strong clinical assessment and care planning skills
- Experience educating patients and caregivers
- Proficiency with electronic health record (EHR/EMR) documentation
- Excellent communication, collaboration, and relationship-building skills
- A compassionate, patient-first mindset and a commitment to delivering dignified, person-centered end-of-life care
