Hospice RN Case Manager

Assess patient preferences, values, and barriers to care while developing individualized plans of care with the interprofessional team. Document goals, communicate updates across the continuum, and evaluate changes in patient conditions to ensure evidence-based practice and quality outcomes. Advocate for patient-family centered continuity through collaborative problem solving and cultural sensitivity. This role supports BJC Home Care's largest regional network, providing skilled nursing and palliative services to thousands of patients in Missouri and Illinois. Candidates must register with the Family Care Safety Registry.
