Hospice RN Case Manager - Bartow / Cherokee Counties

Job Summary
Manage end-of-life patient care for a designated caseload in Bartow and Cherokee counties, including symptom management, pain control, and family education. Observe signs of physical or emotional changes, report unexpected conditions to physicians and the interdisciplinary team, and develop care plans in conjunction with IDG. Teach patients and families on home and assisted living care, perform specific nursing procedures per doctor's orders, and document visits in Wellsky Consolo. Attend bi-weekly interdisciplinary meetings and team conferences while adhering to HIPAA, infection control, and hospice policies. Coordinate care for patients in SNFs or ALFs and respond to crises outside hospital settings.
Required Qualifications
- Graduate from an accredited school of nursing
- Currently licensed as a registered nurse to practice in the state of Georgia
- Minimum of 3 years as a registered nurse
- Active patient bedside care within the past three years
- Must provide evidence of annual TB test and other state required tests or examinations
- Must have current valid driver's license, auto liability insurance and your own reliable transportation
- Must be able to work independently
- Make accurate and at times quick judgements
- Ability to supervise others appropriately
- Ability to respond appropriately to crisis outside of a hospital setting
- Acceptance and adaptability to different social, racial, cultural and religious modes
- Must be able to cope with mental and emotional stress and demonstrate emotional stability
- Must be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids, excrements, adverse environmental conditions and hazardous materials
- Requires considerable physical effort most of the day including, kneeling, squatting, reaching, twisting, climbing, walking, exposure and humidity changes and maximal assist in lifting and/or transferring of a 100 (+) pound patient
- Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position
- Must be flexible, innovative, and possess good interpersonal skills
- Must be available for Monday - Friday, 8:00am-5:00pm
- Must attend bi-weekly interdisciplinary group meetings in the office
- Must be assigned to the Bartow/Cherokee counties of Canton and Acworth
Desired Qualifications
- Experience with hospice a plus
- Have completed or the ability to complete a designated Hospice training program
