Job Summary
Supervise hospice aides and deliver professional nursing care by applying the full nursing process. Complete comprehensive patient and family assessments to determine needs, then develop and implement care plans based on nursing diagnoses. Act as Case Manager when assigned to coordinate patient care for your caseload, while counseling families on nursing requirements and providing appropriate health care instructions. Requires an active RN license in Connecticut, one to two years of acute care experience, and one to two years of home care experience. Offers per diem pay with weekly payment, holiday pay, and 401(k) matching.
Required Qualifications
- Graduate of an accredited school of nursing
- One (1) to two (2) years of recent acute care experience in an institutional setting
- One (1) to two (2) years of recent experience in home care
- Active and unencumbered RN license in Connecticut
- CPR certification
- fully vaccinated against COVID-19
