Job Summary
The Hospice RN Case Manager provides primary nursing case management for hospice patients in home settings, including assessment of patient and family needs and coordination of care with the interdisciplinary team. Responsibilities include determining eligibility, admission processing, planning, implementation and evaluation of the nursing plan; initiating communication with attending physicians and collaborators; conducting regular home visits to reassess needs and update the plan of care; performing clinical pronouncement of death and supporting families; maintaining accurate records and care plans; supervising Home Health Aide delivery of care; participating in team meetings; initiating medical orders to contracted vendors; and fulfilling on-call duties with emphasis on safety, documentation, and quality of care across the patient and family journey. The role requires flexibility, critical thinking, and adherence to department and company policies, with travel to patient homes as needed.
Required Qualifications
- Graduate from an accredited registered nursing program
- Licensed as a registered nurse in the state of practice and in good standing
- Two (2) years experience as a registered nurse in a clinical care setting. Hospice experience preferred.
- Must possess current CPR, First Aid Certification
- Must be knowledgeable of quality assessment and assurance procedures
- Excellent observation, verbal and written communication skills, problem solving skills
- Valid driver's license and proof of insurance is required
- Background check and drug clearance
