Job Summary
Conduct comprehensive patient assessments to identify healthcare needs, develop individualized care plans, and update plans as needed. Provide skilled nursing care, including pain and symptom management, medication administration, and patient education, while coordinating and supervising hospice aides. Communicate effectively with patients, families, and the interdisciplinary team to discuss treatment plans and address concerns, ensuring compliance with regulatory guidelines and documentation requirements. Monitor patients' physical, emotional, and spiritual well-being, offering bereavement support to families. Participate in team meetings to review care strategies and respond swiftly to emergency situations during home visits. Requires a current RN license, minimum two years of hospice experience, and ability to lift up to 50 pounds. Full-time weekday shifts with mileage and cell phone reimbursement.
Required Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Minimum of 2 years of experience as an RN in a hospice or related healthcare setting.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Valid driver's license and reliable transportation for home visits.
- Current CPR certification.
- Knowledge of regulatory requirements and guidelines governing hospice care and documentation.
- Demonstrated commitment to providing compassionate, patient-centered care in a hospice setting.
- Ability to lift and transfer patients, as well as carry equipment and supplies weighing up to 50 pounds.
- Capable of prolonged standing, bending, squatting, and walking during home visits.
- Fine motor skills for documentation and administering treatments.
- Ability to respond swiftly and efficiently to emergency situations.
- Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager.
- Job requires accepting criticism and dealing calmly and effectively with high-stress situations.
- The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without 'giving out' or fatiguing.
Desired Qualifications
- Experience with electronic medical record systems and documentation preferred.
