Hospice RN Case Manager

Job Summary
Serve as the primary RN for your patient and family caseload, completing assessments to determine hospice eligibility and leading the admission process. Develop individualized plans of care that evaluate physical, emotional, social, and spiritual needs while coordinating interventions with the interdisciplinary team. Conduct regular patient visits to reassess needs, update care plans, and perform clinical pronouncements of death with compassionate family support. Maintain accurate documentation and supervise Home Health Aides per federal and state guidelines. Work Monday through Friday from 8 a.m. to 5 p.m. with only two holiday on-call shifts annually.
Required Qualifications
- Graduate of an accredited registered nursing program
- Current RN license in the state of practice
- Current CPR and First Aid certification
- Knowledge of quality assessment and assurance practices
- Strong clinical judgment and ability to make independent decisions
- Compassionate communication skills and the ability to support patients and families
- Valid driver's license and reliable, insured vehicle
