Hospice RN Case Manager

Job Summary
Manage a patient caseload and lead the full nursing process, including assessment, admission, care planning, implementation, and evaluation. Deliver holistic patient care addressing physical, emotional, psychological, social, and spiritual needs while conducting routine visits and updating individualized plans of care. Collaborate with an interdisciplinary team comprising physicians, social workers, and chaplains to coordinate care with attending physicians and external providers. Provide end-of-life care, including clinical pronouncement and family support, while maintaining accurate documentation and supervising Home Health Aides. Work Monday through Friday from 8:00 AM to 5:00 PM with only two holiday on-call days annually.
Required Qualifications
- Active Registered Nurse (RN) license in Ohio (or compact state if applicable)
- Graduate of an accredited nursing program
- Current CPR and First Aid certification
- Knowledge of quality assurance and hospice care standards
- Valid driver's license and auto insurance
