Hospice RN Case Manager

Hospice RN Case Manager responsible for managing a patient caseload and leading the full nursing process including assessment, admission, care planning, implementation, and evaluation. Deliver holistic patient care addressing physical, emotional, psychological, social, and spiritual needs; collaborate with interdisciplinary team (physicians, social workers, chaplains); coordinate care with attending physicians and external providers; conduct routine patient visits and update plans of care; provide end-of-life care including clinical pronouncement and family support; maintain accurate documentation in compliance with state, federal, and company standards; supervise and oversee care delivery by Home Health Aides.
