Job Summary
Develop individualized hospice care plans in partnership with the interdisciplinary team while providing skilled nursing services including symptom management, medication administration, and wound care. Coordinate care with physicians, social workers, and home health aides to ensure seamless patient support, monitor clinical changes, and revise care plans promptly. Maintain accurate documentation, complete OASIS assessments, and lead field staff including LPNs and aides. Participate in IDT meetings and on-call rotation while traveling within the Northern Lawrence coverage area.
Required Qualifications
- Current Registered Nurse (RN) license in Kansas
- Minimum one year of nursing experience
- Current CPR certification
- Valid driver's license
- Reliable transportation
- Auto insurance
- Strong critical thinking skills
- Strong communication skills
- Proficient in EHR documentation
- Ability to lift up to 50 pounds
- Visiting Nurses requires a criminal background check
- Visiting Nurses requires a motor vehicle (MVR) check
