Job Summary
Conduct comprehensive assessments of hospice patients and provide skilled nursing care and symptom management. Develop and implement individualized care plans while coordinating with physicians, social workers, and chaplains. Educate patients and families on disease progression and document all interactions in the EMR system. Participate in interdisciplinary team meetings and ensure compliance with hospice policies and regulatory requirements. Respond to patient and family needs in a timely and compassionate manner across home and facility settings. Requires local travel and prolonged periods of driving.
Required Qualifications
- Graduate of an accredited school of nursing
- Current RN license in the state of practice
- Minimum of 2 years of nursing experience
Desired Qualifications
- Hospice and Palliative Care certification
- At least 1 year in hospice or palliative care
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Proficiency with EMR systems and documentation
- Ability to work independently and manage a caseload
- Compassionate approach to patient and family care
- Must be able to manage stressful situations with empathy and professionalism
- Requires working under some stressful conditions to meet patient needs and regulatory deadlines
