Job Summary
Hospice Registered Nurse Case Manager responsible for performing comprehensive patient assessments, coordinating hospice care with the inter-disciplinary team (IDT), developing and updating individualized POC, ensuring timely execution of physician orders, and providing patient/family teaching. The role includes documenting care activities, maintaining regular updates to the MD and Hospice Manager, supervising home health aide activities, ensuring timely completion of hospice paperwork and recertifications, and collaborating to anticipate and meet patients’ hospice needs while meeting productivity expectations.
Required Qualifications
- BSN or minimum two years hospice care experience preferred
- Minimum of two years of clinical experience
- Current Wisconsin RN license
- Current driver's license
- Able to take on call responsibilities on rotating basis
