Hospice RN Care Manager

Job Summary
Provide compassionate, patient-centered nursing care to patients in homes and care facilities throughout Marysville, Ohio, and surrounding communities. Assess patients and families to develop individualized plans of care, monitor condition changes, and adjust medications, interventions, and visit frequency as needed. Coordinate care with physicians and the interdisciplinary team while educating and supporting patients, families, and caregivers. Complete accurate documentation in the electronic health record and participate in admissions, IDT meetings, and quality improvement activities. Work independently in the field with an 8:00 AM–4:30 PM schedule, including holiday and weekend rotation, requiring a valid driver's license and ability to travel safely.
Required Qualifications
- Graduate of an accredited nursing program.
- Current RN license in the State of Ohio in good standing.
- Current CPR certification.
- Valid driver's license and safe driving record.
- Required auto insurance.
- Ability to travel throughout the assigned territory and safely drive in varying weather conditions.
- Ability to complete required background check and drug screening.
- Basic computer skills and accurate documentation ability.
Desired Qualifications
- Two years of acute care nursing experience.
- Hospice or palliative care experience.
- CHPN certification.
