Job Summary
Assess, plan, implement, and evaluate patient care while directing hospice services for individuals and families in their homes. Build community resources to organize and direct care plans, with an emphasis on community health education. Requires a valid Indiana RN license and previous hospice or long-term care experience preferred. Full-time Monday-Friday schedule with nights free, offering medical/dental/vision insurance, 401k, PTO, and mileage reimbursement.
