Job Summary
Resource RN position in hospice care responsible for cross-team patient and family assessments, planning and updating the plan of care, triaging needs, and delivering interventions across homecare teams. Requires coordinating with the Interdisciplinary Team to meet hospice eligibility, providing patient/family education, supervising CNAs/LPNs, and documenting in the EHR. Day shift (8:30am–5:00pm) with occasional weekends/holidays, 0.7 FTE (28 hours/week); includes a sign-on bonus. ADN is required with BSN preferred; current RN license in Wisconsin; valid driver’s license and vehicle access; strong skills in pain management, education, and care planning within a hospice/palliative care setting.
Required Qualifications
- ADN required; BSN preferred
- Current, unrestricted Registered Nursing License in state of employment
- One-year nursing experience, preferred
- Valid driver’s license, auto insurance, and access to a reliable vehicle
- Must be at least 18 years of age
- Basic Microsoft Office skills
- Intermediate electronic health records skills
- Ability to use proper body mechanics, safe patient handling, and standard precautions
