Job Summary
Provide care and services for a group of patients by visiting them in private homes, Skilled Nursing Facilities, and assisted living facilities. Contribute to the interdisciplinary team by performing psych-social assessments, implementing interventions, and leveraging knowledge of community resources. Manage visits, documentation, and meetings while prioritizing and triaging urgent needs through phone, email, and written communication. Work independently to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership. Maintain a valid driver's license and auto insurance to travel across the service area. Join a Community Health Accreditation Partner certified hospice dedicated to changing expectations of hospice care by increasing clinical visits and lowering clinician caseloads.
Required Qualifications
- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice
- Valid driver's license
- Auto insurance
- Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
- Ability to learn an electronic medical records system
Desired Qualifications
- Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
- Work closely with patients on a 1-1 level but get strong support from a patient care team
- Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations
