Job Summary
Evaluate and admit patients for Hospice services, coordinate care with the interdisciplinary team, and provide education to patients and families regarding hospice philosophy. Conduct ongoing assessments to ensure appropriate initiation of care plans and services that reflect clinical, functional, and psychosocial needs. Serve as a resource for patients and families through the end-of-life process, ensuring quality of life and dignity are maintained. Work Monday through Friday with two holidays annually.
Required Qualifications
- New York State licensed Registered Nurse
- BSN from an approved school of professional nursing
- One year of RN experience as a clinical nurse in a hospital or home care agency
- BSN and a minimum of six months experience in hospital or home care
- Current NYS Driver's License
- Monday - Friday, Days, 2 Holidays a year
