Job Summary
Provide care and services to patients residing in private homes, skilled nursing facilities, and assisted living facilities as a member of the interdisciplinary team. Conduct psych-social assessments, interventions, and coordinate with social workers, chaplains, volunteers, and CNA leadership. Manage visits, documentation, and urgent needs through phone, email, and written communication while prioritizing multiple concerns. Work independently to drive across the service area using reliable transportation and a valid license.
Required Qualifications
- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice
- Valid driver's license
- Auto insurance
- Reliable transportation
