Hospice Registered Nurse (RN)

Provide care and services for a group of patients by visiting them in private homes, Skilled Nursing Facilities, and assisted living facilities. Collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership to contribute to the interdisciplinary team. Conduct psych-social assessments, interventions, and coordinate community resources while managing visits, documentation, and meetings through time management skills. Work independently using phone, email, and written communication to respond to urgent needs by prioritizing and triaging multiple concerns. Requires current RN licensure, a valid driver's license, and ability to learn an electronic medical records system.
