Job Summary
Deliver compassionate, patient-centered end-of-life care in hospice settings by performing comprehensive patient assessments, managing symptoms and pain, and creating individualized care plans. Collaborate with an interdisciplinary team to educate and support patients and families through the journey, while thinking critically to solve real-time problems and ensure thorough, compliant EMR documentation. Work independently within an assigned territory, traveling to patient homes with a current driver's license, dedicating approximately 20% of your day to driving. This role offers a $10,000 first-year success bonus and is supported by a dedicated field education team and leadership focused on mentorship and growth.
Required Qualifications
- Active Registered Nurse, RN license in the state you work
- Two years of RN experience
- Current driver's license
- Ability to spend ~20% of your day driving to/from patient locations
- Successfully complete a drug screen
- Confirm legally authorized to work in the United States
