Hospice Registered Nurse RN - PRN/On-Call

Job Summary
Conduct hospice patient care via phone triage, home visits for changing conditions, and weekend admissions. Perform comprehensive physical assessments and histories to determine needs, then write and initiate plans of care that incorporate palliative nursing actions. Administer prescribed medications and treatments while documenting pain and symptom management outcomes. Re-evaluate patient and family needs regularly to revise care plans as status changes, communicating updates to physicians and seeking orders as required. Provide health care instructions to patients based on assessments. Requires a valid driver's license, local travel capability, and a minimum of one year in post-acute care.
Required Qualifications
- Graduate of an accredited Registered Nursing program
- 1 year of nursing experience in post-acute care setting
- Must be licensed as a Registered Nurse within the state in which working and must maintain licensure
- Valid Drivers License
- proof of insurance
- satisfactory motor vehicle record
- Ability to communicate effectively both verbal and written
- Proficient with Microsoft Office Suite
- Able to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Home Care, Bill of Rights, OSHA, Universal Precaution, AWAIR, Right to know, and SDS, ADA HUD policy and procedures, Fair Housing laws and regulations
Desired Qualifications
- Bachelor of Science degree in Nursing
- One year nursing experience in Hospice
