Conduct comprehensive and ongoing assessments of patients and families to determine hospice needs and psycho-social status. Initiate and revise the plan of care in collaboration with the interdisciplinary group, physician, and family while coordinating services among internal and external providers. Administer medications and treatments as prescribed, providing education and interventions for pain and symptom management to assess patient response.
