Hospice Registered Nurse RN PRN

Conduct home visits to assess terminally ill patients and families, documenting assessments, interventions, goals, and outcomes within the established plan of care. Coordinate total hospice care under the supervision of the Director and interdisciplinary team, including collaboration on clinical plan revisions and continuity with ancillary providers and long-term care facilities. Supervise Hospice Aides and Licensed Practical Nurses while ensuring compliance with Medicare standards, Joint Commission requirements, and agency policies. Participate in on-call rotations for weekend, holiday, and after-hours care delivery across Marengo, Hale, Sumter, Choctaw, Dallas, Greene, and Perry Counties.
