Hospice Registered Nurse RN PRN

Conduct pain management and symptom control for terminally ill patients and families according to the plan of care established by the interdisciplinary team and attending physician. Perform regularly scheduled and after-hour home visits to assess total patient and family needs, document interventions, and coordinate care with ancillary providers and long-term care facilities. Supervise Hospice Aides and Licensed Practical Nurses to ensure compliance with the established plan of care and required documentation. Collaborate on the development and revision of the clinical component of the patient plan of care, monitor patient appropriateness for hospice services, and participate in quality improvement and infection control activities. Provide continuing education on the hospice concept of care to patients and families while maintaining accurate patient chart information and adhering to Medicare Conditions of Participation and Joint Commission standards.
