Job Summary
Triage patient needs over the phone, conduct visits for patients with changing conditions, and complete weekend admissions. Perform comprehensive assessments of patients and families to determine hospice needs, including physical assessments and illness histories. Write and initiate plans of care, regularly re-evaluate patient and caregiver needs, and revise plans as necessary. Develop care plans incorporating palliative nursing actions, administer prescribed medications and treatments, and record symptom management outcomes. Communicate with physicians regarding patient needs, report condition changes, and provide health care instructions per assessment and plan. Requires local travel, a valid driver's license, and part-time weekend availability.
Required Qualifications
- Graduate of an accredited Registered Nursing program
- 1 year of nursing experience in post-acute care setting
- Must be licensed as a Registered Nurse within the state in which working and must maintain licensure
- Valid Drivers License
- proof of insurance
- satisfactory motor vehicle record
- Ability to communicate effectively both verbal and written
- Proficient with Microsoft Office Suite
- Able to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Home Care, Bill of Rights, OSHA, Universal Precaution, AWAIR, Right to know, and SDS, ADA HUD policy and procedures, Fair Housing laws and regulations

Desired Qualifications
- Bachelor of Science degree in Nursing
- One year nursing experience in Hospice
