Hospice Registered Nurse (RN)

Triage, coordinate, and administer hospice care for patients, families, and caregivers by utilizing hospice services and resources. Manage the plan of care from referral through discharge, completing initial nursing assessments and physical histories to determine needs. Implement plans of care in collaboration with the interdisciplinary team, providing teaching on pain and symptom management while administering prescribed medications and treatments. Communicate with the medical director and attending physicians regarding patient needs, participate in team meetings, and maintain accurate clinical notes and timely reports. Provide complete patient discharges, including pronouncement visits, and ensure a safe environment for all patients.
