Job Summary
Provide hospice nursing care to terminally ill patients, focusing on pain management and symptom control within the plan of care. Collaborate with the interdisciplinary team to assess patient/family needs, document interventions and outcomes, coordinate care across settings, supervise hospice aides and LPNs, and participate in on-call rotations. Deliver home visits and coordinate with facilities and other providers to ensure continuity of care, while educating patients and families/caregivers about hospice concepts of care.
Required Qualifications
- Active RN license in the state of application
- Valid CPR
